How to Enter for a Chance to Win
Beginning June 1, 2010 through 11:59 pm on June 27, 2010, visit www.1057thehawk.com to fill out the Participant application and upload a photo of your smile. Or, download the application, complete it and send it along with a recent photo of your smile (head shots are best) to Ron Rotem DDS P.C., 355 Lakehurst Road, Toms River, NJ 08755. Photos should be in .jpg format with a file size no larger than 1MB. All contest entries will become the property of Station and Dr. Rotem and will not be acknowledged or returned.
All applications will be evaluated by Ron Rotem DDS P.C. (“Producer”), and one winner will be selected in Dr. Rotem’s sole discretion. All decisions concerning selection of the participants are final and not subject to challenge or appeal. Winner will be notified by phone and/or email.
Prize
One winner will be selected to win the grand prize of a “Smile of Your Life,” consisting of dental cosmetics (veneers, teeth whitening, crowns, contouring, etc.) required to create a new smile. Specific procedures will depend on the winner’s individual needs. ARV up to $15,000.
Winner is responsible for consulting with Dr. Rotem and his/her personal physicians regarding medical status, health, and any risks posed by undergoing the treatments and procedures included in the Prize. Winner will be required to provide medical information to Dr. Rotem and/or undergo diagnostic testing, on a confidential basis, to determine eligibility for certain procedures. Winner must be an approved candidate for all selected procedures as determined by medical history and physical/psychological exams performed by participating physicians. Additional laboratory and diagnostic studies may be necessary prior to performing the procedure(s), the cost of which (including, without limitation, any pharmaceutical prescriptions) will be the winner’s sole responsibility. If the “Smile of Your Life” procedure is not approved for winner as medically safe, there will be no substitution of prizes. Any additional services or procedures beyond the item(s) included in the Prize will be at the winner’s sole expense.
Non-cash prizes are not redeemable for cash. Producer and Station reserve the right, in their sole discretion, to substitute any prize with one of equal or greater value. Winners may not request prize substitution. Prizes are non-transferable and may not be claimed or used by any person other than the winner. Selected winner must confirm notification within 96 hours, or s/he will be disqualified and an alternate winner selected. Prize must be claimed and used within one year, or it will be forfeited.
Eligibility
All entrants must be at least 18 years old and a citizen of the United States, living or working in Station’s terrestrial listening area. Employees (including their immediate families and members of the same household) of Greater Media New Jersey, its affiliates, promotion sponsors, advertising and promotional agencies who do business with 105.7 HAWK or any of the promotion sponsors, and other radio stations in 105.7 HAWK’s ADI (as defined by Arbitron) are not eligible to participate. The term “immediate family” includes spouses, siblings, parents and legal guardians, children and legal wards, grandparents, and grandchildren, including “in-laws.” One entry per person; multiple entries will disqualify the entrant. Void where prohibited.
In addition, the following eligibility criteria apply: You must not be a candidate for public office and must agree not to become a candidate for any public office for at least one year if you are selected as the winner of the “Smile of Your Life”. You or any of your immediate family members (spouse, ex-spouse, parents, siblings, children) and household members (whether related or not) may not be employees, officers, directors, representatives or agents of Ron Rotem DDS P.C., 105.7 HAWK, or GreenRose Media.
